Any traffic accident that occurs in the state of Wisconsin and involves injury of a person, $1,000 or more damage to any person’s vehicle or property, or damage of $200 or more to state or other government-owned property, other than a vehicle, must complete a crash report. In most cases, if law enforcement officers are called to the scene of the crash, one of the officers will complete the crash report. However, if law enforcement officers are not at the scene of the crash, then you will need to fill out a crash report and submit it to the Wisconsin Department of Transportation (DOT). Many insurance companies also require that you obtain a completed crash report if you or another driver file an insurance claim.

If any driver involved in the accident is uninsured, then there are additional forms that must be completed under Wisconsin’s safety responsibility law. You can fill the crash report form out online on the website of the Wisconsin DOT. You also can upload a diagram of your crash to the website to include with the crash report form by either scanning or taking a photo and uploading it using a camera or smartphone.
You also can purchase and print a copy of a crash report from the DOT that has occurred within the last 90 days. In order to do so, you will need to know the date of the crash, the name and/or driver’s license number of at least one of the drivers involved in the crash, and the DOT crash report number.
